Manager Case Management

ABOUT THE ROLE

Join SMRC within the Diverse Care team and be responsible for overseeing the delivery of high-quality case management services. This is a dual role involving the leading and managing of a team of case managers, providing mentoring, supervision and support in addition to holding complex cases.

  • Full time, ongoing position
  • Salary packaging available

Key responsibilities include:

  • Develop and implement case management strategies, policies, and procedures that align with organisational goals and regulatory requirements collaborate with multidisciplinary teams to coordinate comprehensive Goal Directed care plans
  • Optimise resource allocation and ensure programs and services are culturally safe and responsive to the community’s diverse need
  • Develop and maintain relationships with key referral agencies and stakeholders to improve service coordination and delivery, referral pathways and local collaboration.
  • Ensure compliance with all regulatory requirements and standards particularly the Aged Care Quality Standards

Qualifications:

  • Division 1 Nurse or other Clinical Qualification with minimum of 3 years Management Experience

  • Police Check (not older than 6 months) 
  • Employee Working with Children Check (not expired) 
  • NDIS Worker Screening Check (not expired)
