You will support the Community Stakeholder lead, acting as a connector between primary care services and the Chin community, focusing on improving health outcomes, enhancing service accessibility, and promoting community capacity building.

Bicultural Support Program Worker (Chin Community)

You will act as a bridge between the Chin community and services by utilizing shared lived experience, cultural knowledge, and language skills to facilitate engagement, trust, and safety.

Multicultural Community Stakeholder Engagement Lead

The Multicultural Community Stakeholder Engagement Lead will build and strengthen partnerships between Primary Care Services and multicultural communities, through consultation, advocacy, and identifying gaps for service improvements that will reduce barriers for multicultural communities to access primary care services.

Promote and contribute to the maintenance of elderly people’s independence and quality of life through individual and personalised care.
