Your rights
You have a right to make a complaint about SMRC, a worker or responsible person. This includes making a complaint about them not meeting their obligations. SMRC welcome your complaints and feedback via completing the form below. Both help SMRC understand community experiences and identify opportunities for continuous improvement.
Making a complaint can improve your quality of care and help other people with the same problem. Your complaints can be anonymous if you want, although this will limit our ability to investigate and report back to you.
How to make a complaint
You can choose to make a complaint online, by phone or in writing to SMRC.
Online
You can make a complaint using our online webform. Simply click here. You may need to provide written evidence to us in writing separately to your online form. We will contact you if this is required.
Phone or in-person
You can call us at (03) 9767 1900 or visit our offices at 39 Clow St, Dandenong 3175.
In writing
Please address your complaint to the ‘SMRC Complaints Officer’. Make sure your letter includes:
- your name, address and telephone number
- the date you’re making your complaint
- details of your complaint, including what happened and when it happened (dates)
Interpreter
If you need an interpreter, you can ask us to arrange one when you lodge your complaint.
